HOW TO BOOK YOUR SHORT BREAK ACTIVITY HOLIDAY:

• Please contact our Care Manager, Silvie MacKenzie T: 01479 861285 (and press 2), to enquire about availability and discuss your individual needs
• Complete an Application Form ensuring it is clearly signed
• Post your Application Form to us, together with either full payment of the fee; payment authorisation from the Social Work Department; or by arrangement
• Please note: fees are non-refundable in the event of cancellation less than 3 weeks prior to the visit. 50% can be refunded on production of a doctor's certificate or in similar circumstances.
